Think You Have Sciatica? Why Most Sciatica Advice Is Wrong

“Just stretch it.” “Strengthen your core.” “Rest until it settles.” If you have searched online for sciatica treatment, you have probably heard all three. The problem is that pain travelling down your leg does not automatically mean you have sciatica. Leg pain can come from the lumbar spine, hip, sacroiliac region, muscles or other structures, and true nerve-root-related pain needs to be distinguished from these conditions. A proper clinical assessment—including neurological examination where appropriate—is therefore far more useful than following a generic exercise programme from the internet.
Sciatica needs more than a generic exercise
Even when the symptoms genuinely are sciatica, there is no single treatment that works for everyone. Current NICE guidance recommends exercise and self-management, but also supports manual therapy such as mobilisation or manipulation when used as part of a broader treatment package. The purpose is not simply to “put your back back into place”; appropriately selected manual therapy can help reduce pain and improve movement, making it easier to return to normal activity and exercise.
